There's probably some criteria we need to meet for him to spawn, I want to do some tests to find out what it is but I don't know where to start. Was there anything special that you did on those run throughs?
First play-thru was on normal with bots. Second play-thru was online (normal)
What settings were you playing on and were you playing with bots?
Not only does he have more health, hes immune to fire. But it takes 1 wack from a meele weapon to kill him

He has spawned twice in a row for me. No commands, he was just there, already spawned.
